Personal Outcome Measures®

KenCCID utilizes the Personal Outcome Measures® tool to ensure support and services are truly individual-centered. Caring for individuals with disabilities has come a long way. Days past, care felt more like forcing individuals to comply with a strict regimen. This regimen, of course, catered to the interests of the caregivers. At KenCCID, care for individuals with disabilities centers the interests of the individual.

What are Personal Outcome Measures®?

The Personal Outcome Measures® (POM®) is a person-centered discovery process. In a POM® interview, 21 indicators are used to understand certain outcomes. These outcomes involve choice, health, safety, social capital, relationships, rights, goals, dreams, and employment. 

The Personal Outcome Measures® interview provides a clear pathway to ensure these outcomes are present and can be achieved. The POM® is an internationally-recognized tool that has been an effective data set for valid and reliable measurement of individual quality of life.

How are Personal Outcome Measures® used?

At KenCCID, we use the insight gained during a POM® interview to develop an individual-centered plan. These plans guide our entire spectrum of care and support for individuals with disabilities. 

Why are Personal Outcome Measures® important?

“When people directly describe what they want out of their life, organizations can ensure that services are actually facilitating those outcomes.”

“At an individual level, the Personal Outcome Measures® can be used to inform a truly individual-centered plan, and track progress in implementing that plan. At an aggregate level, organizations can collect Personal Outcome Measures® data to analyze the impact of their supports, identify effective services, and highlight areas where additional work is needed.”

We believe that when we center the needs of the individual, we as an organization can provide an improved quality of care. This is because at KenCCID we’ve stopped to listen to the needs of the individual first, and not just the needs of the organization. 

One size does not fit all

For us at KenCCID, one size does not fit all. We meet individuals with disabilities and their families where they are. First, we listen. Then, we implement a detailed care and support plan catered to their unique needs and desires. 

In this way, KenCCID continues to fulfill its mission to promote dignity and choice to empower Individuals with Intellectual and Developmental Disabilities reach their full potential.
